Ability to print activity forms from the open form

Users are requesting the ability to print completed activity forms. 

Whilst they can use the browser print function for this, having a print button 
on the header of the form would be helpful to users who are unfamiliar with 
browser print functionality.

This has increased in priority due to DoE h/c record keeping requirements and 
the need for project members to be able to view completed activity forms (and 
in locked stages) in a read-only format. Using browser print functionality to 
print read-only pages should be fine.

This is currently possible only via activity listings associated with sites, 
but not from the Plans & Reports listing. Need to support from both places.
