Improve user interface and terminology for Picklists in the BVP

After discussions with Paul Flemons, John Tann and Rhiannon Stevens, existing 
pick list structure can remain, but requires enhancements to the user interface 
to make their use for intuitive.

"Picklist Institution Code" should be renamed to "Collection Code". 

When loading picklists, existing collection codes should be presented in a drop 
down list.

New Collection Codes can be added via an 'Add' button that pops up a 'modal' 
window to collect a simple string

Should also be a way to see what DwC terms have values for each Collection code 
(improve the PickList list UI)

Rules for determining which set of pick list items to use for a DwC term are as 
follows:

  If no Collection code is specified for the expedition, items from the <default> set (no collection code) are used
  If a collection code is specified, and there are picklist items for that DwC term and Collection Code, then they will be used
  If a collection code is specified, but no pick list items for that DwC term exist, values from the <default> set for that DwC term will be used
