Update home page #92

This task is to make the following changes to the home page (first page of the mockup:

  1. Remove the left nav bar and PSIP icon. Note: the nav bar will still be visible used on other pages.
  2. Remove the "Home" text from the top left.
  3. Add the large "Take the survey" button centered under the text. This button will link to the assessment view.
  4. Update the text and layout to match the mockup.
