Identify visits within each patient.

Rule based: Split into new files always between "*****" and "Record date:"

this way, the body of each visit always starts with the date the record was created

only TODO for #20 is to finish implementing getTimeIntervalBetweenVisits() ... should be done quite quickly, will leave that though for later ...
