Add title input #1

Adding a title input for an abbreviation would make this plugin complete.

There is now also example code/functionality for Gutenberg UI autocomplete for acronyms: https://github.com/WordPress/gutenberg/blob/master/docs/designers-developers/developers/filters/autocomplete-filters.md#example

I need to incorporate this, thanks for finding it! I'll try to find a little time soon, because you're right--that's all that's missing from having a standalone, convenient solution for acronyms in WordPress.
