If an incident fails to load, no IMS updates follow #57

When an incident fails to load, such as this sort of error:

2015-08-07 05:24:50.810 Incidents[22273:4789513] [ERROR] Error while attempting incident #11 request: The operation couldn’t be completed. (NSURLErrorDomain error -1001.)

then the failed incident stays in the loading list, which means we never exit the loading state and we never reload again.

The above error isn't all that exceptional; it happens when a request times out, and this is happening reliably to me now when the app launches and it's loading all incidents over a slowish network.
