SpiffyTitles: unusual markup yields incorrect title #60

A previous change made SPiffyTitles look for the last title tag in the document. Some pages apparently use the title tag in places other than the head tag. This can be fixed by targeting head > title instead.

Apparently if the second title tag is blank, it will use that. This is a bug.
