Main changes:

Extend Azure Pipelines CI to also run the tests on a hosted Mac OS agent.
Simplify requirements handling by in-lining requirements.pip into setup.py.
Make bsddb3 always required on MacOS since bsddb doesn't seem to be supported by default.
