Re-injest PPE Output files to remove duplicate entires

Currently for a given SimulationSample the SmokePPESimulationOutputFile table contains multiple entries for most of the files:

[x] Modify the SmokePPESimulationSample#upsertFileTable method to set the id to the filename to avoid duplicate entries.
[x] Drop the current SmokePPESimulationOutputFile table.
[x] Re-ingest files.
