Sanitiser should keep a log of every pass it makes #49

Since the sanitiser is becoming more and more sophisticated, it'd be nice to have a degree of traceability as to what it's actually doing. Adding something into Ctx that lets it build a log of each thing it changes shouldn't be too difficult.
