Issue 209 - README and CONTRIBUTING update

What does this PR do?

Fixes #209 - Provides general updates to README.md and CONTRIBUTING.md to help make things easier for new project participants.

@rhythnic this is a follow-up to some of this work you already completed.

Also, this PR puts CONTRIBUTING.md and CODE_OF_CONDUCT.md back into the base of the repo to align with how we generally manage CableLabs OSS projects on GitHub.
