The documentation contains a link showing how to use this plugin

Read this blog post about how to configure your specific client.

Unfortunately that link is broken and should probably be removed.

It is not yet compatible with version 2 until someone can thoroughly test it: https://github.com/caddyserver/forwardproxy/pull/74

(it's in a branch you can try though!)
