Without other modules enabled (except libWrapper), if I attach a template to an actor, it is not duplicating the actor. So maybe something specific to your setup?
What modules do you have active? Are there any errors reported in the browser console?
It ended up being a midi problem that I believe was Patches within like an hour of me posting this.

When a template is set to be connected to an actor and then you move that actor, it visually duplicates the actor leaving one in the original location. I tested this with the burning hands spell, toggling between measurement controls and back to character controls removes the duplicate image.
