Kingdom Hearts Re: Chain of Memories (NTSC) fails to boot (CDVD plugin fails to boot) #969

1) Did the game ever work correctly (i.e. not have this problem) on the
Official PCSX2 build or an earlier version of PCSX2 playground?
(If so, please specify the latest pcsx2-playground or Official revision
that last worked.)

I have tried this only on R3876 and R3113, both producing the same result.

2) What steps will reproduce the problem?
1. Select the ISO
2. Attempt to load (fast or full)
3. Tada!  Error screen.

3) What exactly happens when you experience this issue (listing any console
errors or screen output you receive)?

Right after attempting to load the iso, I get an error message that says "CDVD 
Plugin failed to open.  Your computer may have insufficient resources or 
incompatible hardware/drivers."  I also get the following console code (I block 
my ISO location for privacy)

4) What version of PCSX2 are you using? On what operating system? Plugins?

I am using both 3876 and 3113.  I use a Windows 7 x64.  These are my plugins 
(using default settings.):

GS: gsdx-sse2-r3878
PAD: lilypad-r3878
SPU2: spu2-x-r3878
CDVD: I use all the plugins with default settings, same result each time.
USB: USBnull
FW: FWnull
DEV9: DEV9null

5) Please provide any additional information below.

Processor: AMD Phenom II X4 810 @ 2.6Ghz
Video Card: Nvidia GTS 450

So far, Kingdom Hearts is the only game that causes this problem.  Dragonball Z 
Budokai Tenkaichi 3 and Sonic Heroes work just fine.  I personally think it is 
just either some problem with my iso, or the emulator just has a funky glitch.

EDIT: This works in the absolute newest SVN revision, 4406 (the latest that I 
could get from EmuCR).  Something must've been fixed.  You might want to update 
the beta download list from the official website.

You almost 100% surely had the iso mounted in a disk emulation software.

No, I made sure to dismount the image.  I didn't change any mounting stuff 
whatsoever between my last failed attempt and my current success.  It was just 
an emulator problem.
