Implement CF7 and create ACF in Appointment Page and few adjustments

1.) Please implement the CF7 2.) Please use CF7 Conditional Fields if necessary 3.) A few changes in input in appointment section

• Rename "Procedure Name Requested*" to How are you feeling?
• Remove Doctor's Request label and input form
• Select Service dropdown must be conditional if selected -> Laboratory Service then a new drop down related to laboratory will show -> Bacteriology -> Blood Banking -> Clinical Chemistry
• Interchange the arrangement of Preferred Date to Call and Time to Call. Preferred Date to Call must be on top.
  • Please Add You, - Preferred Date to Call You. Best Time to Call You.
  • Mode of Payment will be (Cash, Cheque, HMO)
  • Change "Remarks" to "Other Concerns"

Add ACF WYSIWYG for the top messages, as well as WYSIWYG for the information bubble (blue) as well as the text at the bottom.

The CF7 successful delivery message will be "We have received your message, please allow us 24-48 hours to process this transaction.
