(docs) Create basic documentation on how to set up CI providers #67

This pull request contains some documentation about how maintainers can set up builds on appveyor and/or GitHub actions by adding some workflow files as an example. The available environment variables/secrets for each build provider is also being displayed to make it easier for maintainers to know what is available.

NOTE: The TOC was automatically generated by the vscode extension Auto Markdwon TOC, and the markdown comments are related to that.
