Footnotes #76

I needed footnotes in some files. So I added an option to use markdown-it-footnote.

I made the option default off, so to not change anything for existing installations.

Thanks ! You need to install first "markdown-it-footnote" in the node_modules and footnotes display at the end of the PDF as desired. Just what I needed. I hope this commit will be merged into the core.

The feature I have integrated in v2 #53. Nevertheless, thanks for the PR!
