Minor provision doc updates #199

I deployed to AWS yesterday and here are some minor updates I noted. Push back if they're not appropriate because maybe I was just doing something wrong :)

Show example of fab command for AWS
salt-master needs to be included in roles for single-server setup
Show that first deploy needs to be done with root user
Update examples now that all deploy settings are in 1 file
Use Scott's preferred settings for CSR request
