[sp24 recruitment] Adds demographics questions to main application

Why?

As of Fa23, we've found that deliberations consists of a lot of assumptions in identifying the background of various applicants, which leads to an imbalance in the way we assess applicants. The demographic survey is not a sufficient alleviation to this as applicants' responses are anonymous, and do not include additional information like other background of the applicant. Including an optional background short-response question has been underutilized by applicants.

Changes:

This commit adds the following multiple-choice questions to the main application:

• Race
• Ethnicity
• Identity Groups
