Finalizing the Steroid-Induced Osteonecrosis Phenotype

Case Criteria:

  • Presence of ANY corticosteroid (no minimum dose IV, IM or PO ≥ 14 days cumulative) (medication string)
  • NOT presence of Alcohol Abuse (ICD9 code)
  • NOT presence of Sickle Cell (ICD9 code)
  • NOT presence of Gaucher (ICD9 code)
  • NOT presence of Legge-Calve-Perthes (ICD9 code)
  • NOT presence of HIV + (Exclude if value ≠ “undetectable” or is NULL) (ICD9 code) (medication string)
  • NOT presence of Organ Transplant (ICD9 code) (CPT code)
  • NOT presence of specific keywords related to comorbidities (keywords)
  • Presence of aseptic necrosis of bone (query window from steroid initiation to termination + 1 yr) (ICD9 code) OR
  • Presence of specific keywords suggestive of osteonecrosis (keywords)


Control Criteria:

  • Presence of ANY corticosteroid (no minimum dose IV, IM or PO ≥ 14 days cumulative) (medication string)
  • NOT presence of bisphosphonates (pulled list from Wikipedia)
  • NOT presence of Alcohol Abuse (ICD9 code)
  • NOT presence of Sickle Cell (ICD9 code)
  • NOT presence of Gaucher (ICD9 code)
  • NOT presence of Legge-Calve-Perthes (ICD9 code)
  • NOT presence of HIV + (Exclude if value ≠ “undetectable” or is NULL) (ICD9 code) (medication string) (lab code)
  • NOT presence of Organ Transplant (ICD9 code) (CPT code)
  • NOT presence of specific keywords suggestive of osteonecrosis (keywords)

Using the CDC spreadsheet cross-referenced by what we have data for yields the following (I'm not sure we want row 2):

2 56888-1: HIV 1+2 Ab+HIV1 p24 Ag [Presence] in Serum or Plasma by Immunoassay

That being said, we also have data for the following labs that seem very relevant, yet are not included in this spreadsheet:

Which labs should we move forward with?

1) re: bisphosphanates, no. You probably know this, but -onate = -onic acid. Also, whatever the trade name equivalents are for those.

2) We should probably use all 6. Rows 2,3,4 are all yes/no and seem good to use. If I read the phenotype at the top correctly, we exclude patients if their viral load (rows 1,5,6) ~= undetectable. Is that how you read it?

@trinklek, I am reviewing the source_strings for this phenotype (definition here) and found that one did not return a match: "aredia pamidronate". I found the following from CHEMBL:

Yes, these synonyms are accurate. I am surprised to see a string with both the brand and generic in it. I will be interested to see if this happens frequently.

For #1, I wonder if leaving the "-" out makes a difference. If not, BMT might work.

I believe #2 is misspelled. Dissecans vs dessicans.

3, is the 3rd in reference to which version of the test was used? If so, just going with HIV seems appropriate.

Great suggestions, both solutions were tried and did not return a match. I also tried "marrow transplant". Maybe it's OK that this didn't match and we are covered with the ICD9cm and CPT codes the author supplied (see below)? Unless, you think something is missing?

OK, I was legitimately worried that the typo was my fault. Turns, out that "dessicans" is how the phenotype author spelled it and I just copied the spelling mistake. Sort of ironic that even word tried to highlight the typo! 😃

Earlier this year, it was suggested that we focus on the labs listed in a specific CDC Resource and here you can see what I found form digging it through it and querying our data sources. What are your thoughts?

@jwyrwa, I missed one sorry! Any alternative strategies for searching "bowel transplant"? I was thinking I could try "intestine transplant" and "intestinal transplant"?

I like the HSCT idea, though that term includes sources other than the bone marrow, such as peripheral blood and umbilical cord blood. If there's a way to specify the source, then HSCT could work. Otherwise, I would say that those codes that you listed should have us covered.

For "HIV3rd", it looks like it is referring to the third generation of the immunoassay, which is outdated with regards to clinical guidelines. It looks like LOINC only refers to 4th generation. What about "HIV4th" or just "HIV"?

As for bowel transplant, yes, "intestine" / "intestinal" could work. Other alternatives I can think of are specifying which part of the bowel/intestine, divided into small and large, and subdivided into duodenum, jejunum, ileum, cecum, ascending colon, transverse colon, descending colon, rectum, anus. The ascending colon, transverse colon, and descending colon can also all be lumped together as "colon" as well.

OK, for simplicity I will hold off on adding anything else for this term at this point.
