Fixes link picker issues in Umbraco v7.12 #33

Resolves issue #32 meaning the nav item settings dialog did not function in Umbraco v7.12.

This issue was caused by a number of changes to the link picker functionality in the core, including: https://github.com/umbraco/Umbraco-CMS/pull/2702
