Fix helmet #83

Description

Craft appropriate default CSP headers so we can reenable helmet.

Motivation and Context

**Why is this change required? What problem does it solve? Fixes #54

How Has This Been Tested?

Work in progress. This has at least resolved the specific issue in #54

this is a draft, so I need to tell myself to finish this by:

[x] updating docs for the new environment variable to bypass helmet, and the new optional config file
