Support versions #20

Implement version support:

CRDC-H model GDC data dictionary PDC data dictionary Mappings caDSR NCIT

Since LinkML has a code_set_version field, it'd be great if the Terminology Service returned some sort of version for each enumeration. This could be the datetime when the Terminology Service was last updated, the version of the field being used, or some other value that change when the contents of the enum change.
