Add an optional query param for 'thesaurus' #76

Description

If someone is querying the terminology service, they should be able to add a thesaurus param to their query and only receive codes from that source.

http://terminology.ccdh.io/path/to/endpoint?thesaurus=ncit

@ShahimEssaid You mentioned this once, and this is the one thing I messaged you about on Slack and spoke to you about briefly at last Monday's DMH meeting. Did you want to elaborate on this? This seems like a valid request / requirement, but I haven't heard anyone else talking about this requirement for this service.
