Make cmap for each target model #53

For each target standard model make a cmap. The cmap should have boxes for entities and arrows for relationships. Those entities and relationships that appear in the core CRDC-H entities identified during completion of #9 should be included and labeled according to the model documentation. The total number of properties and relationships for each entity should be added, in parentheses, after the entity label. Try to organize the diagram so no arrows cross each other.

The May 2020 deliverable was scoped to include FHIR and BRIDG mappings of a subset of ADM subdomains / entities. As such, I have modified this issue to that scope.
