Created a form template and necessary functions for its execution

REMOVE FORM FROM DATA OPENSEARCH AND POSTGRESQL AFTER TESTING

• Check the form on the demo: static and dynamic
• Make sure that the form opens correctly and has all needed fields (you can check expected field sets in form-data.json)
• Check that the form submits necessary data
• Check that the form has correct required fields and does not submit if a required field is empty

On the dynamic forms, the country dropdown is being cut short to an awkward size. Can you look into this please?

I'm getting an error on form submission for both forms

The payload on submission for the dynamic form has "how-many-machines-do-you-have", I believe this field should be concatenated in "Comments_from_lead_C". This is not the case for static forms though.
