Decapitalise input names
Update hidden inputs

Go to https://maas-io-679.demos.haus/contact-us
Fill in the form and submit it
Check it's submitted and redirect to a thank you page on maas.io
Check the submitted data is on the list in marketo (form ID is 1337)

Thanks @anthonydillon for reviewing! Were you able to see the data on marekto? I still can't find the data I submitted on marketo. Is it okay to merge this PR?

Hi @anthonydillon! I found the issue on sentri and it works now!
