contact form forces US/canadian states when using other countries #692

users trying to contact us on https://maas.io/contact-us are forced to select a north american state or province even when selecting countries outside of north america

@antongisli Do we actually need these fields? They're missing in the contact us modal.

has this been changed already? i only see the modal now when clicking contact us.

There are two separate contact us forms.

The contact us page is linked in the footer The contact us modal is triggered on click on the contact us link in the main navigation and a few other places via contact us link (e.g. Enterprise support for MAAS section on the main page)

I'd suggest to just keep the one on the contact us page and get rid of the modal. I don't see a reason for maintaining two. It will make the job of removing jquery from the site much easier easier, too. Modal is the only place where it's used.

I'd also suggest to simplify the contact us form on the contact us page (to reflect the last page of the modal version where we only ask for first name, last name, and email address).
