chore: use forms for opening GitHub issues #23

Description

This PR does 2 things:

We use forms for opening issue templates, making it more straightforward for users to fill the appropriate content in the right place and to enforce mandatory fields.
We add an "enhancement" form. This allows for users to propose enhancement and admins not having to edit the labels for enhancement type issues.

Notes

The same change will be made in every Telco owned project.
