Wizardry V: cannot get past starting screens / possibly fails to create disk #77

I can not read Japanese, but using Google Translate it seems to me Wizardy V fails to create a user disk and keeps looping through the starting screens.

I have a blank disk image set for FD0. After starting Wizardry V I press M, then Enter, then Y. I land on 'error 76' and after another Enter the game returns to the first screen.

User error or an issue with the emulator?
