The question "Enter your STEP 1 Score (optional) " is supposed to appear for 3rd and 4th year medical students and residents and interns only..
The question "Enter the name of your illness(s)" should only appear for people who are patients or caregivers...
The NPI question should show up for:
I am a practicing physician I am a mid-level provider (nurse, PA, etc) I am in a residency or fellowship program
The question: "In what language do you communicate with your patients? " should appear for everyone who says they are any kind of caregiver...
The questions:
Enter your STEP 2 Score (optional) ?
What medical school did you attend?
What year did you graduate medical school?
What is your primary specialty?
Should appear for practicing physician, resident or fellowship only...
Right now, I can choose "No" I am not a clinician of any kind and "No" I am not a patient or a caregiver... and I am still asked questions that are specific to medical students and questions that are specific to patients and caregivers.
That does not make any sense. My expectation is that you are understanding the purpose of the form, and that you understand what this form is doing: gather relevant information depending on what type of data contributor is being consented. The form needs to make sense.
Please do not close the issue, unless you are confident that the survey form makes sense.

On the final consent form, there are parts of the form that should only show based on what was chosen on the form above.
For instance, school email address and MCAT score should only display if the person filling out the form about has listed themselves as a medical student..
The phrase "(based on choice above)" should be removed from all form elements, and instead they should only show when that choice is taken.
