Update Bootstrap from v3 to v4 #214

There have been some fairly substantial package updates to stable versions. I've started work in the bootstrap-4-upgrade branch. This is not ready for merging into master.

Bootstrap v3 to v4:

This is going to require a lot of changes. The CSS theme that they included is no longer provided.

In addition, many classes have had name changes or have been deprecated completely.

We may need to do this sooner rather than later. There are some known security vulnerabilities with the antiquated 3.X version.
