Calculate Shipping Stopped Working #3

I am being asked to look into a problem for a client. They installed this plugin 2-3 weeks ago and now it is not working. I am getting the following error in Chrome.

Uncaught TypeError: Object [object Object] has no method 'tmpl'

The site is using the theme ‘Jitensha’ with jquery 1.6.2
