Preparing for first reviews #7

  1. Update the README to prepare for the first reviews.
  2. Replace the "use the Incubator" issue template with an author submission checklist.
  3. Add license and code of conduct files.

Note: it will probably be impossible to really test out the issue template until this is merged, but please read through it and we can road test it later.
