Update README #194

📚 Context

We have been making changes to the repo, for example:

renamed the CartesiDApp contract to Application contract, and other DApp related contracts as well
removed History contract
enabled storage of claims in the consensus contract itself

and so on. These changes cause the README to be outdated.

✔️ Solution

update broken links
remove and update outdated texts
update graph if needed

After some thought, I'm keeping the word "Cartesi DApp" in some places of the README, as it's more specific than just "Application". The same applies to the mermaid graph, "Cartesi DApp" is probably better than "Application". Plus, it's showing as "Cartesi DApp" rather than "CartesiDApp", so it probably doesn't need to be the same as the contract name.
