No object to handle subscription cancelled notification #44

What steps will reproduce the problem?
1. Create a subscription
2. Cancel the subscription
3. Log the notifications that the .NET code sample doesn't handle.

What is the expected output? What do you see instead?
Expect an object to exist for all notifications.

What version of the product are you using? On what operating system?
Latest source.

Please provide any additional information below.
Notification is "cancelled-subscription-notification". It is not in the 
apiv2.cs.

I will check to see why this is not in the xsd.
