Move changelog into it's own file #23

The changelog will likely grow over time, adding unnecessary noise to the readme.

This is an opinionated change, close if you prefer to keep as-is.

In my plan the changlog in README is a temporary solution before we start versioning vundo and have release notes. So I won't be too worried about it now.
