Could you provide an example / min steps for reproduction, as it's not clear how I could test this myself if I wanted to.

This is quite a gnarly one to test, as it requires a full smartmedia stack setup. But a basic replication case it to embed a video inside of a file resource activity, and view the file in the activity page. It will sometimes serve through a URL that isnt 1:1 with the DB internals
