we can actually embed the wufoo forms instead of linking to them, so that they show up on the page. I haven't made wufoo forms yet, though, because we still need to decide what information to collect. How is LACCR planning on following up with users who submit contact info?

Ah gotcha - I didn't see his answers to my questions earlier b/c I was on my phone, but now I see it.
I think an alternative phone number is a good idea, b/c in Illinois, it has sometimes been difficult to reach people. I'll make it an optional field though, so as not to prevent anyone from filling out the form.
What do you mean the name of the person seeking expungement and the name of the person inquiring about it? Like, someone looking into expungement on behalf of someone else?
I'm not sure what you mean by the record - do you mean the arrest record? The form shows up in the eligibility question tree right after people find out that they are eligible for expungement, so people probably won't have had a chance to pick up their arrest record
Yep: in case a parent is inquiring on behalf of their 18 year old son, for example.
Record: That is confusing. I'm guessing Josh meant where they were arrested/where their case happened. So something like: "What city were you arrested in?" "Which parish did you go to court for this case?'
added the wufoo forms - left out the question about the court case, since not all users will have gone to court.
Looks great! Good point about the court case--could you put in "If you went to court for this case, what parish was that in?"
Actually, I'll make two forms - all folks who didn't go to court get to the eligible1 page while all folks who went to court get to the eligible 2 page, so it'll be easy to split

alrighty fixed - now http://expunge-la.herokuapp.com/eligible2 has question about court & http://expunge-la.herokuapp.com/eligible1 doesn't
So the eligibility pages (eligibility1 and eligibility2) should have wufoo links, as well as links to the forms. The person who can upload those to the LCCR website is out of town til next week, but do you want to add in the links to that now?
Also: http://expunge-la.herokuapp.com/adultrecord should have a link sending users to http://www.jaclouisiana.org/#!contact/c1z0x
