CudaMiner not working with multiple GPU's (commit version 289eb72fe0da2c4e0534095369f39e4b4b01ac8b) #126

Hi,

under Ubuntu 12.04 using cuda 5.5 & cuda 6.0 this release build correctly and work 5 khash/s faster then the release 2014-02-28. The only issue the recent change do not seems to correctly work with multiple GPU. I also make multiple tests, disabling the nvml & changing the usleep value from 0 to 500 with the same issue, only the first GPU is sending results.

see attached output using the untouched version.
