Add population-adjusted datasets #112

Add population-adjusted datasets (per 1,000, per 100,000, or per million, as appropriate), along with raw values and population values used. Can use most recent population value available in geo for the level of geography.

Could mention the new map files in the geo section README (discussed in the commit comments for https://github.com/ccodwg/CovidTimelineCanada/commit/f4caf08b78ba6fb823162373b3888f5f960b0ab0). The geo data should also be mentioned more prominently in the main project README (#93).
