As far as explicitly licensed data, I think we have:

Ontario: Contains information licensed under the Open Government Licence – Ontario.
Public Health Agency of Canada: Public Health Agency of Canada under the Open Government Licence - Canada.
Quebec: Ministère de la Santé et des services sociaux under the CC BY 4.0.
Population data: Adapted from Statistics Canada, 17-10-0009-01, 2022-03-17 and Statistics Canada, 17-10-0134-01, 2022-03-22. This does not constitute an endorsement by Statistics Canada of this product.
All other data are provided as is by the relevant provinces and territories through publicly accessible websites and datasets. Specific sources are available by cross-referencing the UUIDs in the data download script with the dataset metadata.
The license of the dataset and scripts should be added and clarified.
Additionally, relevant data on the licenses of datasets used to construct the dataset (such as they are available) should be added.
