Hello, As mentioned previously, I took the liberty of converting the solution to SharePoint 2013.

Changes I made: 1) Updated all strings to read 2013 FBA Pack (I hope) 2) Assembly and Feature FBADiagnosticsService Versions are 2.0.0.0 3) Change Password Page is no longer a table-based layout 4) Change Password Page now has a validation summary 5) Added a Style.css in Layouts/15/FBA/Css which currently contains the styling for the Change Password Page (Not the template)
