Remove scan_varying from "Simple" parameters in indexing #55

Scan-varying refinement is almost certainly a Bad Thing to do in indexing as it doesn't refine the underlying static model. Indexing only uses the static model to update re-assign indices at each macrocycle. This option should definitely be removed from the "Simple" parameters in the DUI.
