Upgrade BiopsyReferralPeriod from 1 month to 1 year to account for real-world scenarios in which somebody cannot get a follow-up biopsy in 1 month.

This was causing an error in which patients who received a biopsy more than 1 month after their abnormal screening test were being managed with the wrong risk table, or with no risk table at all.
