Add "Report an Issue" link #22

Adds link for opening an issue. I'd like it to open a specific issue template, but it doesn't work. Opened a discussion here: https://github.com/quarto-dev/quarto-cli/discussions/7203

It looks like that issue has not yet been addressed.

It got tagged with the 1.4 milestone, so hopefully will be fixed in the next version of Quarto

Going to go ahead and merge this. It'll at least link to an empty issue for now and will be fixed when we render it after 1.4 is out
