Define rule: Within a StudyVersion and a codeSystemVersion, there is a one-to-one relationship between code and codesystem. #143

This is to avoid situations where the same code has different decodes or the same decode has different codes. The scope of the rule is within codeSystem and codeSystemVersion, since the one-to-one relationship is expected to hold only if instances of code are from the same source. The scope of the rule is within StudyVersion, as we expect to check data for one StudyVersion at a time.
