Add Rendered Cheat Sheet

The purpose of this PR is to add Firely's rendered cheat sheet to the cds-hooks.org page. To do this, the following changes were made:

Moved the "cheat-sheet" folder under "docs" folder
Added Cheat Sheet PDF (only the second page with actual content, as the first page ad for specific conferences will grow stale)
Added link to sidebar nav after 'Quick Start'
Updated the README wording to add more clarity around purpose of the Markdown version, as well as give credit to Firely for the rendered version

I'll note that I synced with Rien Wertheim last week and got agreement that cds-hooks.org could host this version of the cheat sheet, while Firely would host versions with conference advertisements.
