After #82, #83, #84 add to the tab a form for entering orders. Supports at a minimum:

choice of imaging procedures with a dropdown or typeahead list that includes codes and display names for all CPT codes in #83
choice of reason codes with a dropdown or typeahead list that includes codes and display names for all CPT codes in #84
