Contact us form referrer page or URL for feedback mechanism

📇 User story

As a user, I want the Contact Us form to know what page I'm providing feedback for (which page redirected me to the contact us form) if I click on the "Give us feedback" button under "Help us improve" so that I can provide context.

✅ Definition of Done / Outcomes

[ ] Done when tests are passing, check if the user is giving feedback on a specific page
[ ] Done when PR review is approved
[ ] Done when a design or prototype is approved
[ ] Done when the change is published and available on the documentation website

📜 Acceptance criteria

[ ] A prototype or design is approved - need to check with PM and designers if this is possible
[ ] Clicking on the "Give us feedback" appends the current page's URL as a query string to the contact us form
[ ] The contact us form gets the passed query variable to determine where the user is coming from
[ ] The contact us form displays to the user that they are providing feedback for a specific page
[ ] The contact us form display text similar to "you are providing feedback for "
[ ] The contact us form does not display a "you are providing feedback for " if the user did not click on the "Give us feedback" button, or if the user simply clicked on the "Contact Us" navigation from the top right of the page.

I have thoughts on this.

There a document.referrer property in JavaScript that would need to be passed in a hidden form field during submission. There isn't any guidance on Contact us form. We would love to have depts to start the journey from the contextual footer but after it's up to depts to build their own Contact form to suit their needs.
